Do you carry health insurance? If so, we want to do our best to put it to work for you.

Here at Liberated Rehabilitation, we are in-network with the following insurance plans:

• Medicare

• Blue Cross Blue Shield

• Triwest

• CHAMPVA

• Copperpoint

• Workers Compensation

• Personal Injury Cases

We are constantly looking to expand our network and are actively seeking to do so at all times. Due to a variety of factors, we are unable to contract with some plans at this time. If your plan is not listed here, unfortunately we are not contracted with them, but we are always willing to assist with utilization of out-of-network benefits.

DIRECT PAYMENTS

If you are uninsured or not currently part of our network, we are happy to work with you while utilizing direct payments. Our services are appropriate for use of HSA or FSA, and appropriate receipts (including super-bill) will be provided following treatment. Our fee schedule is available upon request.

Additionally, we have packages available to ease the financial burden of extended care for select conditions, based upon evaluation and prognosis from your Doctor of Physical Therapy. Please inquire for details.

This includes verification of benefits, deductible liabilities, and co-pays. No one likes surprises when it comes to cost, particularly when taking steps to regain your health.
